General reports

Five reports that do not fit the other groups but matter a great deal: two are audit protection, one is marketing, and two are case management.

All five take the standard date range filter and, except where noted, a provider filter — see how every report works.

Missing Progress Notes

Visits in the range that have charges but no SOAP note for the same date — the gap an auditor looks for first.

A visit here means a date of service with at least one charge on the ledger. A SOAP note counts if one is saved for that chart on that date, whichever way it was produced.

Run this before the month closes

Every visit on this list is a service billed without documentation. Federal and state rules require the note to exist for every visit, and carriers can and do demand refunds for services they find undocumented. Clear this report every week; do not let it accumulate.

Diagnosis Statistics

How often each diagnosis has been used across patient diagnosis sets, with the count and the percentage of all diagnoses.

Carriers gather this statistic about you

An unusual concentration on one diagnosis is one of the triggers for a carrier audit. If a single code accounts for a large share of the practice's diagnoses, make sure the clinical findings support it on every chart.

Income Referral

Which referral sources bring in income. One row per patient admitted in the range, grouped by the Referral Type recorded on their Patient Info:

Sort by Collection to see which sources produce paying patients rather than just patients. The report depends entirely on the referral type being filled in at admission — a patient with none is not on it.

Visit Recap

Every SOAP note written in the range: one row per note, with its category.

It is the mirror image of Missing Progress Notes: that report finds charges without a note, this one lists the notes so you can check them against the charges. A clinical-only practice uses it as the visit count for the period.

Attorney Wise Patient

Personal-injury case management: the patients represented by each attorney.

This report has its own filter instead of a provider list. Click the attorney button at the top to open a picker listing every attorney in the Contacts catalog; tick the ones you want, or Select All, then choose the date range. The range filters on the patient's admission date.
